The Transition programme aims to provide professional care and practical supports to to adults with physical, sensory, and intellectual disabilities and their families living in CHO 9 area. The programme supports adults to live their life in accordance to their own needs and wishes in line with the HSE New Directions Framework.

The age eligibility criteria range from 18+ (school leavers).
We provides a range of social, physical, education, occupational and recreational activities. Individualised supports, activities are provided whichare person centred and in line with HSE New Directions Framework. The Transition programme implement Gaisce and ASDAN education tools with the opportunity to progress to QQI. Adults are provided with a keyworker when they begin the programme to ensure that there supports are person centerd.
The CRC receive referrals from the HSE Occupational Guidance, Adult Day and Rehabilitative Training Services.
