What is your experience of moving from paediatric (child) to adult health care services (transition of care)?

Transition of care is the planned move of a young person’s care from a paediatric (child) service to an adult health care service. The HSE are currently reviewing the transition of care young people receive when moving from paediatric health services to adult health services.

They want to ensure the transition of care meets the needs of young people and their families, carers and friends. To do this They need to understand what it is like to go through transition of care so that we can continually improve the transition of care.

They are inviting young people aged 18-26 years who currently are in the process of or have already transitioned from a paediatric to an adult health care service and their family members, carers or friends (18 years or over) to complete a Your Voice Matters questionnaire.
